Overlooked at Old Trafford, Adored in Naples

McTominay was a Manchester United academy product who came through the club’s system, but he never fully convinced successive managers he deserved a central role. He was regularly used as the deepest midfield option at Old Trafford, a job that limited what he could show going forward, and by the summer of 2024 United were willing to sell him to Napoli. At Napoli, manager Antonio Conte handed him a more advanced role, closer to the number 10 position McTominay had long played for Scotland, and even used him on the left of the attack. The change unlocked a different player. McTominay created far more chances than he had in his last season at United and started scoring goals with a regularity nobody at Old Trafford had seen from him. He finished the title-winning season with 12 Serie A goals, breaking a Scotland goalscoring record in the competition previously held by Denis Law, and became the first Scot to be named Serie A’s Footballer of the Season.

The clearest illustration of what McTominay meant to Napoli came on the final day of that Serie A season. Napoli beat Cagliari 2-0 to seal the Scudetto, and it was McTominay who opened the scoring, setting the title party in Naples in motion. It fit a pattern from the closing weeks of the campaign, when he started the scoring in three consecutive league matches in April and set up both goals in a decisive draw with Genoa. Napoli limped over the line in the run-in, but McTominay was one of the main reasons they got there at all, finishing the season two goals behind the club’s top scorer while playing much of it in midfield rather than as a recognised forward.

A Rockier Season Under a New Manager

Napoli could not defend that title. They finished as runners-up the following season, and the club responded by sacking Conte and turning to Massimiliano Allegri for the new campaign. It has brought a turbulent summer, with Romelu Lukaku, Giovanni Simeone and Miguel Gutierrez all leaving, and Napoli bringing in Rasmus Hojlund, another former Manchester United forward, on a permanent deal, alongside Chelsea defender Benoit Badiashile on loan. It is a stark contrast to the confidence of the title-winning season, and puts extra scrutiny on Allegri as he tries to blend a rebuilt squad in his opening weeks in charge.

Even before his heart condition was made public, McTominay had been managing a physical issue. Allegri said after the Como defeat that his midfielder “had some problems with blisters, so he’s been training on a limited basis,” adding that the club hoped to have him back to full fitness as soon as possible. Days later came the more serious news.

Surgery Confirmed, Then Carried Out

Napoli released an official statement explaining that, “following the onset of a mild benign arrhythmia, Scott McTominay will undergo corrective surgery using a cardiac arrhythmia ablation (PFA) in the coming days. He will be available again after the international break.” The procedure, an ablation to correct the irregular heart rhythm, has now been carried out at a hospital in Manchester. Napoli have described the operation as a complete success, and McTominay is expected to rest for two weeks before resuming training, with a return realistically pencilled in for after Scotland and Napoli both come out of the international window.

The timing costs both club and country. McTominay will miss Scotland’s Nations League fixtures against Slovenia twice, Switzerland and North Macedonia, and Napoli must get through matches against Serie A champions Inter, Arsenal in the Champions League, Bologna and Fiorentina without their midfield anchor. In his time at Napoli, McTominay has made 82 appearances for the club, scoring 27 goals, in addition to 73 caps and 15 goals for Scotland, numbers that explain why his absence is felt so quickly whenever he is missing. If his recovery goes to plan, he could realistically be back in contention for the visit of Frosinone shortly after the international window closes.

What Comes Next for Napoli’s Talisman

Medical opinion around the condition has been reassuring. A mild, benign arrhythmia of this kind is described as relatively common in professional athletes and not something expected to threaten a career, and Napoli’s own statement set out the surgery as corrective rather than a sign of anything more serious. That will offer some comfort to a club already contending with a shaky start to the post-Conte era and to a player who has become central to how Naples sees itself on a football pitch, in a way few outsiders arriving from England ever have.

There is also a wider pattern here that Manchester United supporters will recognise with some frustration. McTominay is not the only academy graduate or former squad player thriving away from Old Trafford while the club itself searches for stability under Ruben Amorim. Watching a player who was rarely trusted in an advanced role at United collect a league title and an individual player of the season award in his first season in Italy, and then keep producing at a similar level a year later, says as much about how he was used in England as it does about what Conte and now Allegri have asked of him in Naples.

For Allegri, the timing is unwelcome regardless of the long-term prognosis. Napoli need their best players available as they try to find consistency, and McTominay’s attacking output from midfield has been difficult to replace in his short spells out of the side. For McTominay himself, the surgery is a pause rather than an ending. A player who spent years being asked to sit deep and do the unglamorous work at Old Trafford has spent the past two seasons proving he can do far more than that, and a fortnight in recovery will not undo what he has already shown in Naples. The goal against Cagliari that helped win Napoli a title said more about what he offers now than any injury update ever could.
